Office Policies

Cosmedica Dermatology does not participate in any health insurance plans and is considered out-of-network. We do not bill health insurance plans or submit claims on behalf of patients. We can provide documentation for a patient to obtain reimbursement directly from their private health insurance plan. Cosmedica Dermatology patients are not able to submit any claim for Medicare reimbursement.

We make no assurance of reimbursement. Health insurance plans vary in their coverage of out-of-network treatments and may provide only partial reimbursement. Please note that health insurance plans do not cover procedures that are not medically necessary, which includes all cosmetic procedures.

Health insurance may be used to cover prescription medications and laboratory tests. Self-pay rates for these services are available to our patients, if this option is preferred.

Payment in full is due at the time of service. We accept cash, credit cards (Visa, MasterCard, American Express, Discover), debit cards, and Apple Pay. Personal checks are not accepted.

In some cases, patients may be able to use a flexible spending account (FSA) for services not covered by their primary health insurance plan. Patients should contact their FSA provider to determine if FSA payment for uncovered services is possible. We do not participate in CareCredit or other healthcare financing programs.

Cosmedica Dermatology requests that patients provide an active credit card on file for confirmation of appointments and use at each visit. The credit card must be in the name of the patient or the patient's authorized representative. Credit card information is securely stored by Cosmedica Dermatology’s payment processor.

Credit cards on file are used only for medical and/or cosmetic dermatology services rendered on the day of service, deposits for services agreed to be rendered at a future date, cancellation and no-show fees, and any additional purchases or services. If the credit card on file is declined for any reason, additional declined card charges may apply.

Cancellations within 48 business hours of an appointment and no-shows will incur a service fee that is automatically charged to the credit card on file. This service fee is $175 for medical appointments and $250 for surgical and certain cosmetic appointments.

For cosmetic consultation appointments, a non-refundable deposit of $250 is due at time of booking. Cancellation in less than 48 business hours or no-show of a cosmetic consultation appointment will result in forfeiture of this deposit, and a new $250 deposit will be required to reschedule.

Certain cosmetic procedures require a non-refundable deposit of 20% of the procedure cost. Cancellation in less than 48 business hours or no-show of these cosmetic procedure appointments will result in forfeiture of this deposit, and a new 20% advanced deposit will be required to reschedule.
